The GLP-1 aftermath nobody warns you about [CASE STUDY]

My client Ruffin is 66 years old.

He spent the last year on a GLP-1. Lost weight, but along with the fat went a significant amount of muscle. By the time he came to me he was in what most people in the industry call "skinny fat." Lighter on the scale, but softer, and feeling weaker than when he started.

Less energy. Less strength. A body that didn't match how he expected to feel after a year of serious weight loss.

This is the part of the GLP-1 conversation nobodies talking about.

The medication does its job. The scale moves. But if you're not deliberate about protecting and rebuilding muscle during and after — you can end up smaller without being fitter. Ruffin experienced that firsthand.

So we got to work.

The goal wasn't weight loss.

He'd already lost weight. The goal was body recomposition: simultaneously rebuilding the muscle he'd lost while continuing to drop the stubborn fat that remained. At 66. Four weeks in.

Here's what's actually driving it:

We added carbohydrates back strategically.
After a year of suppressed appetite on a GLP-1, Ruffin was severely under-fuelling. His body didn't have the raw material to build muscle even if training told it to.

We reintroduced carbohydrates at the right times — around his training sessions, giving his muscles the fuel they'd been starved of. He's now eating more food than he has in over a year. And the fat is still coming off.

We built a strength training program around what he had access to.
Home gym and Planet Fitness. No excuses, no overcomplication. A structured resistance program focused on compound movements, progressive overload, and giving his body a clear signal: BUILD. At 66, the training stimulus doesn't need to be extreme. It needs to be consistent and progressive.

We addressed his sleep.

Sleep is where muscle is actually built and fat is actually released. After a year on a GLP-1, his sleep quality had suffered. We made it a non-negotiable priority. That one shift alone changes the hormonal environment his body is operating in overnight.

4 weeks in:

  • 3lbs of body fat lost
  • 1lb of lean muscle gained
  • Eating more food than he has in over a year
  • Getting stronger week over week
  • Energy returning

That's body recomposition at 66 years old. In a month.

What Ruffin's result tells you:

It doesn't matter how old you are. It doesn't matter what you've tried before or what the last year looked like. The body responds to the right inputs at every age.. it just needs the right signal, the right fuel, and enough recovery to actually do the work.

The gap between where you are and where you want to be is almost never about effort. It's about the approach.

Ruffin is proof of what four weeks of the right approach looks like.
